As a nationally certified Care Manager, I specialize in professional in-home assessment that helps families understand the medical, psychological and social needs of their aging loved ones. I provide a detailed Plan of Care that includes recommendations and referrals of providers to assist and support older adults desiring to age in their own homes or assisted care facility. We also customize services to include home and/or hospital visits, attendance at medical appointments and oversight visits to care facilities. Our goal is to support independence and monitor quality of life. We also provide telephone and e-mail consultation regarding aging and long term care issues.

3. SCHEDULE AN IN-PERSON CONSULTATION. We often get calls asking if a friend or family member can attend a private consultation. Our answer is always "yes." It often makes the hourly rate easier to handle and having others with similiar needs in the discussion only increases the pool of ideas and interventions available.
